Please fax the appropriate documents to: (561) 581-5221 scan and email to: mlodge@lodge-co.com (HR Designee) Please watch the WVPP training video and take the quiz, also required by California is to watch the Sexual Harassment video. Take time to go through all of the policies and procedures below.
ONBOARDING REQUIREMENTS Send all documents to mlodge@lodge-co.com
Please retain your copies that you submit.
3. Resume - provide copy in liu of employment application
4. Copy of PT / OT License - California (PTA / OTA)
5. Copy of your errors and ommissions insurance
6. Auto Insurance - Provide a certificate of insurance with Excel-Lin as additionally named
insured
7. Current CPR card (pease send to mlodge@lodge-co.com)
11. Policies and Procedures read and sign and send just the signature page
12. WVPP Workplace Violence Prevention Prgram - read the WVPP policy, watch the
training video and take the quiz. This is required every year per SB 553
13. Sexual Harrasment Training (take course and provide certificate to HR - Required by
California (required every two (2) years.
